...the Transition Concept

Transition has three (3) major components:

  1. Consider goals for life after high school and develop a long range plan to get there.
  2. Design the high school experience so that students gain the skills and competencies needed to achieve his/her post-school goals.
  3. Identify and link students and families to any needed post-school services, supports, or programs before the student exits the school system.

...Transition as part of the IEP

For each student with a disability, the following MUST be a part of the IEP process.
Focus on the student's strengths.
Begin the IEP meeting with acknowledgement of the student's post-school goals or vision.
Build an IEP that will help a student "get there"!

At age 14....

  1. Actively involve the student in the IEP process.
  2. Base the IEP on the student's needs, preferences, and interests.
  3. Determine the student's post school goals.
  4. Identify the student's transition service needs.
  5. Develop transition programming at age 14 to revolve around the student's courses of study.

At age 16...

  1. Actively involve the student in the IEP development.
  2. Base the IEP on the student's needs, preferences, and interests.
  3. Refine the student's desired post-school goals.
  4. Review the student's transition service needs and adjust to meet desired post-school goals.
  5. Develop a statement of needed transition services.
